COMMUNITY SUPPORT
ABC Human Services offers Community Support services to both adults and children with a diagnosis of mental illness or with a co-occurring developmental disability. Our Community Support services promote and assist in the development of skills, interpersonal and community coping skills, symptom reduction, and responsibilities needed to function successfully in the family and/or community.
Community Support is the “clinical” home of the individual and coordinates all treatment needs of the individual which are identified in the individual’s Person Centered Plan.
OUTPATIENT THERAPY
ABC provides therapy to adult, adolescents, children, couples, and families. Therapy is based on the Family Systems Model. It is “Solution Focused and Strength Based”. We look for each persons’ innate strengths instead of their problems. We understand that individuals are the experts in their lives and know what works best for them. The goal of therapy is to assist individuals in identifying their assets and utilizing them to develop effective methods to live life to the fullest. Through a supportive relationship with their therapist, individuals are to experience the success of having improved relationships and stronger natural support systems. We respect the individuals’ right to self determination and values. The development of goals involves the client and is based on their individual needs.
DIAGNOSTIC ASSESSMENT
ABC provides Diagnostic Assessment services which includes gathering information related to the individual’s health and behavioral health history; developmental, social, and psychological histories; and identification of the individual’s strengths and weaknesses. The Diagnostic Assessment report determines appropriate services that the individual can benefit from and is the basis for the development of the Person Centered Plan.
CAP SERVICES
ABC provides a variety of services to individuals who have been approved to receive funding from the Community Alternatives MR/DD Medicaid Program. Services provided by our agency include Home and Community Supports, Supported Employment, Personal Care Services, etc. All services provided are included in the individual’s approved Plan of Care.
DEVELOPMENTAL THERAPY
We provide Developmental Therapy both at the professional and para-professional level. ABC staff assists the individual in acquiring skills not gained during developmental stages of their life. At ABC we teach individuals to advocate for themselves. We whole-heartedly believe that our goal in providing services is for the consumer to learn to become as self-sufficient as possible in the home, job, school, and/or the community.
